Expired checks

I get a lot of expired checks at work that have to be reissued. I've lost a check before that was for a small amount of money. But sometimes I get checks worth over $500. How do you lose that? Is it just carelessness or what? I guess I'm just so picky about my money that I always know where every little penny is.
